In the world of general dentistry, two procedures often carry an unfair reputation: root canals and extractions. At Pacha & Bijan Dental, we want to change that narrative. These procedures are not the "cause" of pain; they are the "solution" to pain caused by deep-seated infection or trauma. Our priority is always to save your natural teeth whenever possible, but when a tooth's health is beyond repair, we provide gentle, expert care to resolve the issue and protect the rest of your mouth.
Root Canal Therapy: Saving the Natural Tooth
A root canal becomes necessary when the "pulp"—the innermost part of the tooth containing nerves and blood vessels—becomes infected or inflamed. This is often caused by deep decay or a physical crack that allows bacteria to enter the tooth's center.
- The Process: We carefully remove the infected pulp, clean the interior of the root canals, and seal the space with a biocompatible material.
- The Result: The pain, which is often a throbbing or sharp sensation, is eliminated immediately. Because the tooth is no longer "alive," it becomes more brittle, which is why we almost always follow a root canal with a protective crown.
When Extractions are the Safest Choice
While we strive for tooth preservation, there are instances where removing a tooth is the healthiest option for the patient. This might include:
- Severe Bone Loss: If advanced gum disease has destroyed the bone supporting the tooth.
- Impacted Wisdom Teeth: When third molars cannot erupt properly and threaten the alignment or health of neighboring teeth.
- Orthodontic Preparation: Occasionally, a tooth is removed to create space for a healthier alignment via Invisalign.

We understand the anxiety associated with these procedures. At Pacha & Bijan Dental, we use advanced local anesthetics and a compassionate approach to ensure you are completely comfortable throughout the process.
