Urgent Interventions: The Science and Safety of Dental Extractions in Maitland

23 April 2026

The Emotional Weight of Tooth Loss

There is no "minor" surgery when it happens in your mouth. Whether it's a wisdom tooth causing pressure or a tooth fractured during a weekend outing at Lake Destiny, the prospect of a Dental Extraction can be daunting.

At Pacha & Bijan Dental, our mission is to preserve teeth whenever possible. However, when a tooth becomes a liability—harboring a systemic infection or damaging neighboring teeth—an extraction is the most compassionate and medically sound choice.

The Clinical Science: Atraumatic Extraction Techniques

The "old way" of pulling teeth involved heavy forceps and significant force. Today, we practice Atraumatic Extractions.

1. Preserving the Alveolar Ridge

The bone surrounding your tooth is very thin (the "Bundle Bone"). If this bone is damaged during extraction, it makes future dental implants much more difficult. We use instruments called periotomes to gently sever the periodontal ligaments that hold the tooth in place, allowing the tooth to be lifted out with minimal disruption to the socket walls.

2. The Physics of Luxation

Rather than "pulling," we use the principles of levers to "luxate" or wiggle the tooth. This expands the socket just enough to break the vacuum seal of the root, ensuring the root doesn't fracture during the process.

3. Infection Neutralization

If a tooth is being removed due to an abscess, the surrounding tissue is often acidic, which can interfere with local anesthesia. We utilize specialized buffering techniques to ensure the area is profoundly numb before we begin.

The Patient Journey: From Pain to Relief

  • Emergency Triage: For our patients in Altamonte Springs and Casselberry, we prioritize same-day emergency appointments.
  • Profound Anesthesia: We ensure you are 100% comfortable. We don't start until you are ready.
  • The Procedure: Most extractions take less than 15 minutes of actual clinical time.
  • Socket Preservation: As discussed in previous guides, we often recommend "bone grafting" at the time of extraction to keep your options open for an implant later.
  • Recovery: We provide a specialized "healing kit" including instructions on avoiding "Dry Socket" (the loss of the blood clot).

Frequently Asked Questions: Dental Extractions

  • How do I avoid a dry socket? Avoid straws, smoking, and vigorous rinsing for 48 hours. The goal is to keep the protective blood clot in the socket.
  • When can I eat after an extraction? Stick to soft, cool foods (like yogurt or smoothies) for the first 24 hours. Avoid anything crunchy that could get stuck in the site.
  • Does a wisdom tooth always need to come out? Only if it is impacted, causing decay on the 2nd molar, or if there isn't enough room to keep the area clean.
  • Will it leave a gap in my smile? We will discuss bridges, implants, or partials before the extraction so you have a plan for restoration.
